ATi release a driver every month, 9.9 is currently out, and it should be 9.10 in the next week or so. I have found 9.3 to be the most stable driver on the non-dx10 cards. I have not had a problem...

The HDDs are in the lower chamber, the optical drives were under the fan, so there was just nothing there at all, just a clear tunnel for them to pass through.

I have no idea at all, unless the motherboard controller told it to spin up faster than recommended, which seems unlikely? I think it must be down to stress?

Its a NZXT fan, it came with the Whisper case review.

It looks like both have came off at the exact same spot.

Everything else, luckily is fine =) It was just two blades which fell off. Just before it happened, the fan was spinning at its max RPM, which is 1550rpm.
